Blue Cross and Blue Shield of Minnesota
Members are rewarded for their healthy habits.
If it’s offered with their health plan, the BluePrint for Health®
fitness discounts program credits members fitness center account up to
$20 per month for membership fees. All they have to do is work out at a
participating fitness center the required number of times each month,
depending on their plan.
Who is eligible?
Blue Cross and Blue Shield of Minnesota members can visit
myBlueCross in the member self-service center at
www.bluecrossmn.com to find out if they are eligible.
Members can click on the “Plan Details” tab and select “Health Programs
and Discounts” and finally, select “find a fitness center.”
Members
must be at least 18 years old to participate in the fitness discounts
program.

Active members of fully insured group plans and most
individual purchasers of Blue Cross plans like Aware Care and
Medicare supplement plans are eligible to participate. Members of a
self-insured group plan that’s administered by Blue Cross may also
be eligible if their employer offers the program.

A maximum of two qualifying adult Blue
Cross members per household may participate and receive the up to
$20 credit.
How it works

If members are eligible, they’ll see a
fitness symbol in the upper right corner of their Blue Cross member ID
card.

They present the member ID card with the symbol to a
participating fitness center to enroll in the program.

Every time they work out, their visits are tracked. Once the
member worked out the required number of times in a month, they will
receive up to $20 credit toward their fitness center membership fees.
(Up to $40 per household for adults covered under each plan.)

Members will continue to get the discount
every month that they are eligible and meet the visit requirement.

If members cancel their fitness center
membership, the fitness center discontinues participation in the program
or they cancel Blue Cross medical coverage, they will forfeit any
unapplied
credits.

United Health Care

In
January 2007 United Health Care Health (a division of United Health Care) began
its Frequent Fitness Club Credit Program.
When
United Health Care rolled out it chose to begin with one employer group. Target Nationwide began April
1st.
Eligible participants:
Two adults (18 yrs of age or older) insured through United HealthCare.

Target Employees can receive up to $20.00 monthly
reimbursement on their membership if they work out 8 or more times
per month. Target employees will be reimbursed directly back into
their Target payroll checks.

UCare Activity Network

UCare
for Seniors can get fit and save $20 per month!
Eligible participants:
UCARE for Senior and UCARE Secure members insured through UCARE MN.
If you have more eligibility questions, please contact
UCARE Customer Service at 1-877-523-1515.

Each individual participating in the program must work out 8 times
or more per month to earn the reimbursement (credit).

Each member is responsible to be certain that their usage is
tracked. Make sure your member check-ins are counting at your club.

Participants may elect to pay membership fees by electronic transfer
(EFT) or annual pay in full. The EFT accounts will be credited to
reflect the $20. Annual pay in full members are encouraged to apply
the monthly credits to their renewal. If they cancel prior to
renewal they forfeit those credits. (Great retention tool!) You
may need to tailor this policy to meet your club’s needs.

Members MUST complete the UCARE enrollment form and provide you with
a copy of their UCARE insurance card.

South Country Health
Alliance (SCHA) is excited to reward its members for
taking the initiative to live an active and healthy
lifestyle. Through the Be Fit program, SCHA members can
receive up to $20 off their health club membership for
exercising 8 days per month.
Eligible Participants:
How it Works:

Eligible members
must present their SCHA identification card to the
health club, where they will complete the Be Fit
enrollment form.

Health clubs will
track member attendance at the club, and eligible
members will receive up to $20 off their health club
membership fees for exercising at least 8 days
during the month.

If the member is
not enrolled with SCHA during the month they
exercise, he/she is not eligible for the $20 Be Fit
reward during that particular month.

Regency Managed
Group
The Health for Wealth
Rewards Program offers our associates a discount on their gym membership
fees. Starting January 2009, eligible associates can work out a minimum
of 6 times per month and receive up to a $20 credit on their gym
membership account.
To be eligible,
associates must be on our health insurance plan. An associate plus one
dependent per household can receive up to $40 discount if they each work
out a minimum of 6 times per month. Associates and the dependent must
be 18 years or older (younger than 23 if a child). The associate must
present their health insurance ID card to the health club along with a
letter to verify their eligibility to participate in this program.
